Acne

Acne is one of the most common skin conditions that occurs when hair follicles become plugged with oil and dead skin cells. The causes are multifactorial, which include stress, genetics, hormonal, diet, medications, environment and certain medical conditions.

Causes & Pathophysiology​

  • Increased sebum production
  • Alteration in follicular growth and differentiation
  • Colonization by P.acnes bacteria
  • Inflammation and increased immune response

Treatment

Prompt treatment of acne is necessary to prevent scarring. Delayed treatment or misuse of skincare products may lead to severe scarring. Here in UMH, we provide a stepwise approach to treating acne and scars as follows:

Dull / Uneven Skin Tone​

Healthy, glowing skin is essential to enhance our appearance. In the normal aging process, there will be a decrease in dermal thickness, loss of collagen and elastin fibres, and a reduction in the number of eccrine glands, which leads to the loss of water-retaining capacity of the skin.

Factors that may speed up the aging process include genetic predisposition and external causes such as smoking, stress, lack of sleep and sun exposure.

Wrinkles​

Wrinkles are defined as creases or furrows in the skin surface, due to epidermal thinning, decrease in chondroitin sulphate and deposition of abnormal elastic tissue in the papillary dermis.

There are two types of wrinkles- dynamic and static. Dynamic wrinkles appear during facial expression, while static wrinkles are permanent creases in the skin.

Pigmentation

Skin pigmentation is one of the major concerns among Asians due to reactive melanin production and frequent sun exposure. UVA rays penetrate to the dermis, causing extracellular matrix breakdown and thus leading to aging. UVB rays can cause sunburn and increase the risk of skin cancer by inducing DNA mutations via the p53 tumour suppressor gene.

Types of Pigmentation

  • Epidermal – freckles, lentigines, café-au-lait macule
  • Dermal – Nevus of Ota, Nevus of Ito, Melanocytic nevus, blue nevus
  • Mixed – PIH, melasma

The golden ratio of the nose is 1 to 1.618 which means if the width of the nose is 1, the length of the nose should be about 1.618. Nasolabial angle is formed by the intersection of the two lines passing through the lower edge of the nose (the columella) and the edge of the upper lip. A range of nasolabial angle from 95-115 degrees is considered desirable in women, while 90-95 degrees is preferred in men.

Scars

Scars are your body’s way of healing after several layers of your skin have undergone trauma. Whether the result of injury, surgery, or severe acne, it can be a challenge to find effective scar treatment. Although there’s no way to make a scar disappear completely, there are ways to diminish it. The right treatment for a scar would also depend on the type of scar you have.

Common Types of Scars

  • Keloid scars
    Firm and either pink or dark red, keloid scars form as a result of extra collagen under your skin. Keloid scars also extend beyond the area of the initial injury
  • Hypertrophic scars
    Look similar to keloid scars but do not go beyond the affected area
  • Contracture scars
    Commonly happens after severe burn. It is often deep and can tighten the skin, sometimes restricting movement 
  • Acne scars
    Textural changes and indentations left on the skin as a result of a serious acne breakout

Tattoo Removal

There is no reason to live in regret and embarrassment over an unwanted tattoo any longer. Through a series of treatment sessions done monthly, Pico laser offers a solution to effectively remove a tattoo of different colours. The pico laser treatment works by breaking up the pigmentation into small particles without affecting the surrounding skin, which essentially is removed via the lymphatic system of our body’s immune system.

Cellulite

Cellulite, also known as orange peel skin, is a term for the formation of lumps and dimples in the skin. It could affect both men and women, but it is more common in females due to the different distribution of fat, muscle, and connective tissues. The exact cause of cellulite is unknown, but some of the factors that appear to be linked are hormonal factors, aging, genetics, dietary and lifestyle changes.

Hyperhidrosis

Hyperhidrosis, or excessive sweating, is a condition where there is sweating beyond the normal needs of the body. The sites most commonly affected are the palms (palmar hyperhidrosis), feet (plantar hyperhidrosis) and armpits (axillary hyperhidrosis).

  • Over-the-counter antiperspirants and deodorants
  • Laser hyperhidrosis
    The treatment uses laser light to permanently destroy sweat glands by heating the sweat gland tissue without damaging the surrounding tissue.
  • Botulinum toxin A (BTA) injection
    Consists of proteins that permanently block the nerve endings that supply the eccrine glands, thus preventing the glands from producing sweat. Treatment involves injections into the affected areas and needs to be repeated every 4-5 months.

Hair Removal

Excessive body hair can be distressing to some people, while some prefer to remove certain parts of the body hair for self-satisfaction.

In UMH, we are using long pulsed Nd-YAG laser for hair removal. The pros of laser hair removal include less pain, a longer-lasting effect, and fewer sessions needed. By using a laser, fine hair removal like facial and upper lip hair removal can be done easily and quickly as well.

Hair Loss Treatment​

Hair loss, or in medical term known as alopecia, occurs when normal hair growth cycle is disrupted due to some factors, such as genetic predisposition, hormonal changes, stress, or secondary to certain medical conditions.

A hair growth cycle consists of three phases. During the anagen phase, hair grows actively. In catagen phase, hair stops growing and separates from its follicle, which is the structure beneath the skin that holds the hair in place. While in telogen phase, the follicle rests for two or three months, and then the hair falls out. Most people lose 50 to 100 hairs per day as part of this natural cycle.
